痴呆性高齢者の生活環境を評価するために、高齢者の感情と行動に着目し、生活環境の影響について行動分析的手法による事例検討を行った。施設に入所する痴呆性高齢者1名を観察対象者とし、ビデオカメラを用いて2時間毎に10分間、5回の観察を行った。10秒間1コマとする時間間隔記録法によって得たデータを分析した。結果は以下のとおりである。1)「喜び」では観察対象者・援助者相互に定位や発語が観察された。「満足感」では頭をゆっくり上下させる行動が観察された。「不安/恐れ」では処置を受ける他の高齢者への定位が観察された。2)観察セッション中に援助を行った援助者は5名であった。「喜び」は3名の援助者が介助した際に観察され、援助者の対象者への定位(出現率88.9〜64.3%)は周囲への定位(出現率0.0〜57.1%)よりも多かった。残る2名では対象者への定位(出現率63.3%、44.1%)は周囲への定位(出現率86.7%、52.9%)よりも少なかった。3)観察対象者では特定の音楽に合わせて頭をゆっくりと上下させる行動が観察された。観察対象者の感情や行動は、援助者や他の高齢者の行動、音楽等の生活環境によって影響されると考えられた。本研究方法は痴呆性高齢者の生活環境の評価に活用できると考えられた。 |

To evaluate living environment for elderly with dementia, with a focus on the emotions and behavior of the elderly person, a case study of the effects of the living environment was conducted by behavior analysis. The subject was an elderly institutional resident with dementia, who was recorded on video for ten minutes every two hours for five observations in total. Data were collected with interval recording in which ten seconds was taken as one frame, and then analyzed. The following results were obtained : 1) The mutual orientation and language between the subject and carer were observed in "Pleasure". The subject's behavior of slowly nodding the head up and down was observed in "Contentment". The subject's orientation to other elderly persons receiving treatment was observed in 'Anxiety/fear". 2) There were five carers who provided care in the observations. "Pleasure" was observed when assistance was provided by three of the carers. The orientation of these carers toward the subject (ratio 88.9%-64.3%) was greater than their orientation toward the surroundings (ratio 0.0%-57.1%). With the other two carers, the orientation toward the subject (ratio 63.3%, 44.1%) was less than that toward the surroundings (ratio 86.7%, 52.9%). 3) The subject was observed to slowly nod his head up and down when specific music was played. The emotions and behavior of the present subject was thus affected by the living environment, such as the behavior of the carers and other elderly, and music. The present method may be applicable in the evaluation of living environment for elderly with dementia. |
