| 内容記述 | 目的:外来受付における患者・家族の自己申告による感染症の症状・徴候に関する問診票が、感染症患者の早期発見に有用かどうかを明らかにすること。 方法:2009年10月～2011年3月迄。A病院外来の初診患者を対象に、問診票による感染症に関連する症状・徴候の自己申告と、受診日から2か月以内の診療録から収集した診断名(感染症/非感染症)を患者IDで照合・解析した。 結果:有効回答23,065人(回収率87.0%)。感染症と診断されたのは1,398人。感染症の症状・徴候に関連する自己申告有りは、有意に感染症の診断の割合が高かった(P<0.001)。自己申告による感染症の検出は、感度76.7%、特異度80.5%、陽性予測力は20.3%、陰性予測力は98.2%であった。 考察:感染症の症状・徴候に関連する自己申告による問診票は、有意に感染症患者を早期発見できるが、自己申告の20.3%のみが真の感染症であった。問診票を改善させる余地があることが示唆された。 |

| 内容記述 | Objective: To evaluate the effectiveness of a questionnaire on symptoms and signs of infections self-reported by patients and their families at outpatient units for early detection of infections. Methods: Patients who made an initial visit to the outpatient unit of hospital A between October 2009 and March 2011 were subjected for this study. At the outpatient check-in of this hospital, patients who were suspicious of being infected and their families were asked to complete the self-report questionnaire on symptoms and signs of infections. Items reported on the questionnaire and diagnosis of whether being infected or not-infected obtained from medical records within 2 months of the initial visit were verified by patients' identification number to conduct tally and analysis. Results: Valid responses were 23,065 (response rate: 87.0%). 1,398 patients were diagnosed with infections. Patients who reported symptoms or signs of infections had significantly higher rate of being diagnosed with infections than patients who did not report neither symptoms nor signs (p<0.001). By the self-report, the detection of infections had 76.7% for sensitivity, 80.5% for specific degree, 20.3% for positive-predictive value and 98.2% for negative-predictive value. Discussion: Our study found that the self-report questionnaires at outpatient settings could contribute to detect patients in early stage of infections. However, only 20.3% of patients who reported symptoms or signs of infections were truly infected. Therefore, It was suggested a need to improve the self-report questionnaire. |
