| 内容記述 | 2型糖尿病患者の首尾一貫感覚を高める支援内容の構造を明らかにすることを目的とした。方法は、首尾一貫感覚を高めると看護師が認識している支援内容を明らかにするために先行して行ったインタビュー調査の結果と、首尾一貫感覚を高める介入内容を明らかにするために先行して行った文献検討の結果を統合し、その結果をAntonovskyの記述を参考に構造化した。その結果、患者の準備状態や療養行動を支援者が理解し、かつ首尾一貫感覚の概念理解を深める関りや、参加者間の関係づくりを促す働きかけがあること、病気への生活を患者と共に振り返って事実を共有したり、支援者としての立場で問題発見や改善を協働して支援することがあること、患者が目標をもって今後の療養生活に意味を見出しながら療養行動を行っていけるように支援することが明らかとなった。これらの支援内容には段階が見出された。 | |||||||||

| 内容記述 | We aimed to elucidate a structural framework to support enhancement of sense of coherence (SOC) in patients with type 2 diabetes mellitus (DM). We constructed the framework by consulting Antonovsky's salutogenic model to combined results of previous investigations. The previous investigations were an interview survey targeting nurse awareness of SOC-enhancing support and a literature search targeting SOC-enhancing interventions. We established the following elements in the first level of the structural framework: supporter/caregiver understanding of patient arrangements and care actions, the relationship to deepening conceptual understanding of SOC, and promoting relationshipbuilding between the people involved. For the second level, we identified a common ownership of reality which involves reflecting with the patient on living with DM, and collaborative support which enables problem discovery and remediation from the perspective of the supporter/caregiver. For the third level and above, we identified support which enables care provision while finding meaning in a life of ongoing treatment and imbues patients with a sense of purpose. | |||||||||
